Band 6 | 22.5 hours per week | One year fixed term contract to cover maternity leave

We are seeking a motivated and compassionate Band 6 Registered Mental Health Nurse (RMN) or Registered General Nurse (RGN) to join our established Dementia Crisis Support Team (DCST) serving the communities of Basildon, Brentwood and Thurrock.
The Team provides crisis assessment and intervention for people experiencing a deterioration in cognition, behavioural and psychological symptoms of dementia, carer breakdown, or changes in physical health that place them at risk of hospital admission. The team offers comprehensive assessment and support for up to six weeks and works collaboratively with local partners to ensure individuals receive the right care at the right time.
Main Responsibilities
As a Band 6 Nurse within the Dementia Crisis Support Team, you will:
- Undertake comprehensive assessments of patients presenting with dementia-related crises.
- Develop, implement and review person-centred care plans.
- Deliver evidence-based interventions to support patients and carers through periods of crisis.
- Assess and manage risk, including safeguarding concerns.
- Support hospital admission avoidance and facilitate timely discharge from services where appropriate.
- Work collaboratively with GPs, Memory Services, Community Mental Health Teams, Acute Trusts, Social Care and voluntary sector partners.
- Provide education, advice and support to families, carers and care home staff.
- Act as a care coordinator where appropriate.
- Maintain accurate clinical records and contribute to service development and quality improvement initiatives.
- Participate in duty and triage functions within the team.

What We Are Looking For
Essential Criteria
- Registered Mental Health Nurse (RMN) or Registered General Nurse (RGN) with current NMC registration.
- Significant post-registration experience working with older adults, dementia, mental health or community services
- To be able to effectively travel throughout the large geographical area
Desirable Criteria
- Experience within dementia services, crisis teams, community mental health or older adult services.
- Knowledge of Mental Capacity Act, Care Act and safeguarding legislation.
- Non-Medical Prescribing qualification or willingness to develop specialist skills.
- Experience supervising junior staff and students.

Service Hours
- Monday to Friday: 9:00am 7:00pm
- Weekends and Bank Holidays: 9:00am 5:00pm

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.
Probationary Period
This post will be subject to a probationary period.
High Cost Area Supplement - Fringe
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (minimum payment of £1,346 and a maximum payment of £2,270 per annum, pro rata for part time)
