The CMHT Forensic Interface Team

The CMHT Forensic Interface Team is a unique and exciting pilot project, integrated within the East CMHT Hub and dedicated to supporting service users returning from Secure Care facilities who require ongoing community-based support and service users within existing CMHT teams, who would benefit from intensive and specialist support. The Forensic Interface MDT team will work independently but alongside the wider CMHT staff team, and will include input from Forensic Psychiatrist and Psychologist, community-based nurses and support staff with access to other community resources and specialists as needed.

About the Role

The successful candidates will need an appreciation of the challenges that many service users face when transitioning from secure care back into the community and those living in the community that require more intensive support and engagement strategies. The core task, from an OT perspective, would be to assess the impact that mental illness, trauma, learning disabilities or autism and long-term institutionalisation have on functioning and to develop plans, in collaboration with the MDT and service users, to address these.

Responsibilities

  • Provide a professional consultative role within an MDT.
  • Screen new referrals to the team as a member of the MDT.
  • Use professional-specific practice models and assessments to formulate collaborative goals and clinical outcomes for our service users.
  • Deliver group and individual intervention in the community to promote life skills and support transition.
  • Liaise with care providers to ensure treatment plans are maintained to support community living.
  • Adhere to clinical, operational and procedural policies and processes.
  • Engage in service development.
  • Support staff well-being.
  • Consult with community agencies to develop community-facing pathways and opportunities for our service users.
  • Engage our experts by experience in co-production.
  • Maintain effective record keeping and outcome/activity data.
  • Care coordination or key practitioner role with a small caseload of service users.
