Occupational Therapist – Home Pathway

Band 6 £45,953 - £54,254 (including Healthcare Additional Annual Salary – HCAS) per annum

Permanent | Full-time (37.5 hours per week)

The role is based within Bromley’s Home Pathway Service, where occupational therapists work collaboratively across health and social care settings, including:

  • Community-based health and social care teams
  • Voluntary sector and public service providers
  • In-patient and broader community therapy services

You will drive independence through rehabilitation and enablement interventions, delivering evidence-based occupational therapy within home environments—with a focus on goal-orientated treatment planning.

Main Responsibilities

  • Conduct complex assessments and treatment planning, evolving care strategies aligned with patient-centred goals.
  • Lead education and training for patients and families, alongside service development initiatives.
  • Supervision of junior therapists and team caseload management to uphold best practice.
  • Appraisal of team members, managing sickness absence and return-to-work policies.
  • Coordination of discharges from the Home Pathway Team, working alongside multidisciplinary teams.
  • Contribute to consultant-led MDT discussions, ensuring therapy input aligns with clinical and strategic priorities.
  • Service improvement initiatives, reviewing current Occupational Therapy offerings and innovating new pathways.
