Band 6 Team Leader - 35 hours per week Term Time Only

This role will involve providing professional and clinical leadership to the special school nursing staff, ensuring that they are engaged with and adhering to local policies, guidelines, and expectations. This is in addition to acting as a first line manager to members of the special school nursing team and engaging with them to deliver a high standard of care.

The Continuing Care Schools Team is made up of the Team Leader position alongside two registered nurses and a team of Band 3 Children's Support Workers.

The Continuing Care Schools Team is an integral part of our wider Children's Complex Care Service which also includes the Long Term Ventilation, Continuing Care, and Swanbourne Gardens Respite Service. As a team we strive to provide the children we support with high standards of care.

Responsibilities

  • To assess, plan, implement and evaluate holistic specialist nursing care in conjunction with the child / young person / family and carers.
  • Ensure that nursing care is safe, effective, and caring and is consistent with clinical guidelines.
  • To act as a source of specialist advice for children, young people, and families in relation to their condition.
  • Provide specific specialist education and training programmes and use information packages for children, young people, families, and lay carers, facilitating the development of self-care skills and knowledge.
  • To work with the multi-disciplinary team to develop and implement plans of care to enable children / young people to be cared for in the community.
  • To follow Safeguarding Children policies and guidelines and participate in monitoring and interventions to meet the needs of children and young people.
  • To conduct health assessments and develop child specific health plans ensuring that children with special medical/nursing/health needs have their needs met within the school setting to reach their full potential in school. This will include delivery of clinical procedures.
