Band 7 Children's Continuing Care Nurse Assessor

Band 7 Children’s CHC Nurse Assessor H ਨੂੰ ਢчным

We are currently recruiting for an experienced Band 7 Children’s CHC Nurse Assessor to support an NHS Integrated Care Board in London.

This is an excellent opportunity for an experienced Continuing Care professional to join a busy team responsible for assessing, reviewing, and coordinating care packages for children and young people with complex health needs.


Key Responsibilities

  • Undertake Children’s Continuing Care (CHC) assessments and reviews, adhering to the National Framework and local policies.
  • Assess the healthcare needs of children and young people with complex health needs.
  • Produce high-quality assessment reports and recommendations.
  • Participate in multidisciplinary team (MDT) meetings and case discussions.
  • Collaborate with families, healthcare professionals, education providers, and local authority colleagues.
  • Lead care planning and ongoing review processes.
  • Maintain accurate and timely documentation.

Essential Requirements

  • Registered Nurse with an active NMC (Nursing & Midwifery Council) registration.
  • Previous experience undertaking Children’s Continuing Care (CHC) assessments.
  • Strong understanding of Continuing Care processes and frameworks.
  • Experience working within NHS services.
  • Excellent communication, report writing, and stakeholder engagement skills.
  • Ability to work autonomously and manage a varied caseload.

Contract Details

  • Role: Band 7 Children’s CHC Nurse Assessor
  • Location: London
  • Rate: £300 per day
  • Contract Period: Until September 2026
  • Working Arrange Nmnt: Full-time contract
