Clinical Care Coordinator – South Coast Nursing Homes

Based in Worthing, West Sussex, South Coast Nursing Homes is a successful, family-owned care home company with 9 homes across East and West Sussex, providing 411 beds with 24-hour, high-standard care for residents in both short- and long-term stays.

Reporting to the Home Manager, this hands-on role involves supporting an experienced clinical team in delivering planned, proactive, and reactive care tasks. The team is dedicated to upholding professionalism, respect, and quality care in a safe and supportive environment.

Fulfilling this role requires relevant qualifications, strong nursing experience, and a commitment to delivering exceptional resident care.

Job Responsibilities

  • Maintain high standards of care – Ensure the health, safety, and welfare of all residents.
  • Champion resident autonomy and choice – Support individuals in exercising their freedom of choice.
  • Uphold dignity and respect – Promote a culture where staff treat all residents and colleagues with respect and professionalism.
  • Leading the care team – Assist the Home Manager in managing the team on duty, ensuring collaborative working for high-quality resident care and clear communication.
  • Engage with families and relatives – Build strong relationships with families, advocates, and next of kin (NOK), delivering updates on residents’ physical and emotional well-being.
  • Comprehensive documentation – Complete records in a concise, timely, and accurate manner, including the creation and review of personalised care plans in response to changing resident needs.
  • Medication management – Administer, order, store, and manage medication compliantly, following legal, professional, and company policies.

Essential Skills, Qualifications & Knowledge

  • Experience in a nursing role
  • Qualified to degree level or above
  • Registered with the NMC, with an active PIN

Desirable Skills

  • Ability to prioritise tasks effectively for optimal outcomes
  • Strong verbal and non-verbal reporting skills
  • Team management experience
  • Clinical experience in areas such as:
    • Geriatric care
    • Frailty management
    • End-of-life care
    • Dementia support
