About the Role
We are looking for a compassionate, enthusiastic, and reliable Cancer Support Worker to deliver person-centred practical and emotional support for individuals affected by cancer. Your responsibilities will include:
- Conducting holistic needs assessments
- Coordinating care, referrals, and support pathways
- Signposting patients and families to relevant services to ensure timely, safe, and equitable access to resources across Somerset

As part of the Cancer Support Worker team, you will act as a single point of contact for people living with and beyond cancer, providing:
- Information, emotional support, and practical assistance
- Support for tasks under the Cancer Personalised Care and Support (PCS) Programme, including:
- Holistic Needs Assessments
- Health and Wellbeing Events
- Treatment Summaries
- Remote Monitoring Systems

- Full-time (Yeovil District Hospital)
- Part-time (Yeovil District Hospital)
