Care Broker (Adult Social Care & All Age Commissioning)
Directorate: Adult Social Care & All Age Commissioning Section: Commissioning Location: Civic Centre, Stoke-on-Trent Grade: Level 7 Salary: £31,022 – £33,699 per annum Hours: 37 hours per week Working Pattern: Open to both internal and external applicants

The Role: Care Broker
As an active part of the Commissioning Team, you will occupy a unique position bridging assessed need and service delivery. While commissioning defines strategic frameworks, your role is to implement them by securing individual care packages that align with assessing priorities.
Key Responsibilities
- Brokerage and Service Coordination:
- Source and arrange appropriate care/support packages that match assessed needs
- Collaborate with social workers, health professionals, and care providers to ensure seamless support
- Negotiate cost-effective care solutions while upholding quality standards
- Secure timely hospital discharges by arranging suitable care*
- Monitor provider capacity and local market trends to address gaps in services

- Operational Delivery:
- Maintain accurate records and ensure compliance with policies, procedures, and contracts
- Manage referrals, prioritising urgent cases efficiently
- Resolve placement challenges and escalate risks when necessary
*Practical experience and diplomacy are key—this requires negotiating with both providers and;

- Market Awareness: Knowledge of Stoke-on-Trent’s care providers, their services, and capacity constraints.
- Problem-Solving Under Pressure: Finding swift, effective solutions in complex situations.
- Attention to Detail: Adherence to audit trails, compliance, and contract terms.
- Collaborative Mindset: Partnering across health and social care teams.
