We're looking for an organised, proactive Care Coordinator to join our Operations team and help deliver outstanding care experiences for our clients, families, and professional carers.


The Role

This is an exciting opportunity for an experienced Care Coordinator with a background in live-in care, domiciliary care, home care or community care.

You’ll be responsible for:

  • Coordinating care packages
  • Managing care schedules
  • Supporting carers
  • Communicating with clients, families, and stakeholders
  • Ensuring continuity of care across your caseload

This role would suit someone currently working as a:

  • Care Coordinator
  • Care Scheduler
  • Care Planner
  • Care Administrator
  • Care Operations Coordinator
  • Field Care Coordinator
  • Home Care Coordinator
  • Domiciliary Care Coordinator
  • Live-in Care Coordinator
  • Care Consultant

Key Responsibilities

  • Coordinate live-in care packages across your allocated caseload
  • Manage client and carer rotas, schedules, and shift changes
  • Facilitate new care package starts and care transitions
  • Support continuity of care through effective planning and communication
  • Serve as the first point of contact for clients, families, and professional carers
  • Manage incoming calls, emails, and correspondent enquiries
  • Maintain accurate records using digital care management systems
  • Assist Care Managers with package coordination and operational planning
  • Escalate safeguarding concerns, complaints, and operational risks at the appropriate time
  • Collaborate with healthcare professionals and internal teams
  • Participate in the out-of-hours on-call rota after probation

Requirements

  • 2+ years' experience within Health & Social Care
  • Previous experience as a:
    • Care Coordinator
    • Care Scheduler
    • Care Planner
    • Care Administrator
  • Experience in live-in care, domiciliary care, home care or community care
  • Excellent organisational, planning and communication skills
  • Experience in coordinating care rotas and supporting care packages
  • Strong customer service and relationship management skills
  • Experience using digital care management systems
  • Knowledge of CQC standards, safeguarding, and care compliance
  • Ability to prioritise multiple tasks in a fast-paced environment
  • High attention to detail and excellent record-keeping skills

Desirable

  • Experience coordinating live-in care services
  • NVQ Level 3 in Health & Social Care
  • Experience with Airtable or similar care software
  • Experience with intercom, WhatsApp Business, or customer support platforms
  • Previous experience within a private-pay or premium care provider
