At Westwood Care and Support Services Yorkshire Ltd, we're looking for a compassionate, organised and proactive Out of Hours Care Co-ordinator to join our dedicated team. This is an excellent opportunity for someone with a background in care who enjoys problem-solving, making decisions, and ensuring people receive outstanding support when they need it most.

As our Out of Hours Care Co-ordinator, you'll play a vital role in ensuring the safe and effective delivery of care outside normal office hours. You'll be the first point of contact for our care teams, service users, families and healthcare professionals, helping to resolve issues, coordinate care and provide reassurance whenever it's needed.

You'll be joining a well-established, privately owned care provider delivering high-quality domiciliary care services to older people and children across the local community. Our people are at the heart of everything we do, and we're committed to supporting our colleagues with excellent training, ongoing development and a friendly, approachable management team.

Previous on-call experience is welcome but not essential, as full training and ongoing support will be provided. What matters most is your care experience, your ability to remain calm under pressure and your commitment to delivering exceptional person-centred care.

What you'll be doing

  • Coordinating the safe and effective delivery of care outside office hours.
  • Working evenings, alternate weekends and Bank Holidays on a planned rota (Week 1 Friday PM to Monday AM, Week 2 Tuesday PM to Thursday AM) averaging 29.5 hours per week.
  • Responding to telephone calls from service users, families, care staff and healthcare professionals.
  • Assessing situations, resolving issues and making informed decisions in line with company policies and procedures.
  • Recording accurate information and maintaining clear documentation.
  • Providing emergency care cover at short notice when required to ensure continuity of care.
  • Monitoring electronic call monitoring systems to ensure care visits are delivered safely and on time.
  • Completing thorough handovers to the office team to ensure seamless continuity of service.
  • Building positive working relationships with colleagues, service users, families and external professionals.

About you

We're looking for someone who:

  • Has at least 2 years' experience working within Health and Social Care.
  • Is passionate about delivering high-quality, person-centred care.
  • Is confident working independently and making decisions under pressure.
  • Has excellent communication, organisational and problem-solving skills.
  • Can prioritise a varied workload and adapt to changing situations.
  • Is committed to providing outstanding support to both service users and colleagues.

Essential requirements

  • Full UK driving licence and access to a vehicle.
  • NVQ Level 2 (or equivalent) in Health and Social Care, with willingness to work towards Level 3 if not already achieved.
  • Minimum of 2 years' experience in a care setting.
  • Must live within a 20-minute drive of Hull to respond to service needs when required.
