Care Coordinator

Care Co-ordinator — Job Advert

Hours: Monday–Friday, 40 hours per week Contract: Permanent Salary: £27,000 per annum

At Choices Homecare, we’re looking for a proactive and organised Care Co-ordinator to join our domiciliary care service. This vital operational role involves scheduling, assessments, care planning, quality assurance, and team support, giving you full ownership of the care journey for individuals.

The Role

As a Care Co-ordinator, you’ll:

  • Build and manage rotas to ensure safe and consistent service delivery.
  • Complete initial assessments, risk assessments, and ongoing care plan reviews in line with CQC requirements.
  • Support quality assurance through audits, spot checks, and observations.
  • Supervise, develop, and support care team members.
  • Monitor ECM KPIs, ensure alerts are addressed promptly, and maintain accurate, compliant documentation.
  • Collaborate with the Local Authority on new care packages and changes.
  • Resolve day-to-day service queries professionally and efficiently.
  • Participate in the on-call rota for out-of-hours support when needed.

Experience & Skills

We’re looking for someone with:

  • Experience in domiciliary care, coordination, or scheduling.
  • Strong organisational skills to manage changing priorities.
  • Confidence in assessments, care planning, and record-keeping.
  • Excellent communication skills and a team-focused approach.
  • High attention to detail and a commitment to safe, person-centred care.
  • A full UK driving licence and access to a vehicle.
