Care Coordinator - Caremark Kirklees

Location: Huddersfield Area

Join our award-winning team at Caremark Kirklees. Working closely with the Registered Manager and field team, you will coordinate care services, manage staff schedules, maintain high standards of person-centred care, liaise with customers and professionals, support recruitment and onboarding, ensure compliance with company policy and CQC requirements, and participate in the on-call rota.

About The Role

Key responsibilities include:

  • Create and maintain staff rotas using scheduling software
  • Respond promptly to customer and staff enquiries
  • Monitor and manage care delivery to ensure service standards
  • Liaise with customers, families, healthcare professionals and local authorities
  • Support recruitment, onboarding and retention of care staff
  • Maintain accurate records and ensure CQC/compliance adherence
  • Provide emergency cover and resolve scheduling issues
  • Participate in the on-call rota as required

Essential Requirements

  • Experience in care coordination, scheduling or care administration
  • Excellent organisational and time-management skills
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment
  • Good IT skills and experience with scheduling systems
  • Understanding of domiciliary care and person-centred practice
  • Full UK driving licence and access to a vehicle

Desirable Criteria

  • Experience in home care or health & social care settings
  • Knowledge of CQC regulations and compliance
  • Level 3 Health & Social Care qualification (or willingness to work towards it)
