Care Coordinator – Faversham

We are looking for a dedicated and organised Care Coordinator to join our Faversham home care team. This is a rewarding and fast-paced role where no two days are the same, and you’ll play a vital part in ensuring our service runs smoothly while delivering high-quality care to our clients.

As a Care Coordinator, you’ll be at the heart of the service—supporting both our care team and the people we care for. You’ll need to be a strong communicator, a natural problem solver, and someone who thrives on organisation and teamwork.

Working Hours & On-Call

  • 37.5 hours per week - Monday to Friday 9am to 5pm
  • Participation in the on-call rota (1 in 2 weekends and 1 evening per week) is required
  • Additional on-call payment

About The Role

This role is ideal for someone who is already working as a Care Coordinator, or for an experienced Field Care Supervisor, Assessor, or Senior Care Worker who is ready to take the next step in their career.

Key Responsibilities

  • Planning and scheduling care calls, creating efficient rotas using the Nourish system
  • Carrying out care assessments and regular customer reviews
  • Supervising and supporting care staff, including supervisions, spot checks, and competency assessments
  • Assisting with recruitment, including interviewing new care staff
  • Supporting with medication processes, including MAR chart audits and transcribing
  • Completing ECM system reconciliations
  • Acting as a key point of contact for client queries and concerns
  • Supporting the development of training materials and content
  • Delivering Care Certificate training (following “train the trainer” completion)
  • Maintaining accurate training records and compliance via Nourish
  • Participating in the on-call rota
  • Supporting with general office duties as required

  • Previous experience in health and social care (essential)
  • Strong organisational and communication skills
  • Ability to prioritise workload and respond to changing situations
  • A supportive and team-focused approach
  • A full driving licence and access to your own vehicle
