Care Coordinator

Salary: From £27,000 + bonus & shared on‑call (DOE) Hours: Full‑time, Monday to Friday plus on call

Apollo Home Healthcare is seeking an experienced Care Coordinator to support the smooth delivery of person‑centred homecare across the Northwest. You will manage rota planning, care package coordination, and act as a key contact for clients, families, and professionals.

This role is based in our Heywood office with regular regional travel. A Full UK Driving Licence and your own vehicle are essential.

Duties and Responsibilities:

  • Rota management and scheduling for care teams
  • First point of contact for clients, families, and healthcare professionals
  • Monitoring care delivery to ensure compliance with care plans
  • Attending multi‑agency meetings with ICBs
  • Supporting the shared on‑call rota (no hands‑on care)

What You Need:

  • Full UK Driving Licence and own vehicle
