Job Description
What you’ll do
As a Care Coordinator, you'll play a key role in ensuring the smooth day-to-day running of the Extra Care Scheme. You'll coordinate and update care rotas, ensuring every tenant receives high-quality, person-centred care while responding effectively to changes in staffing or care needs. You'll be the first point of contact for tenants, their families and healthcare professionals, building positive relationships and supporting the delivery of outstanding care.
You'll provide day-to-day support to the care team, assist with inductions and mentoring, maintain accurate care records, care plans and risk assessments, and ensure compliance with CQC standards. Working closely with GPs, district nurses, social workers and housing colleagues, you'll help coordinate care and respond to changing needs. While the role is primarily office-based and supervisory, you may occasionally provide hands-on care to support the team when required.

Qualifications
What you’ll Bring:
- Previous experience working in care, ideally in a Senior Care Worker, Team Leader or Care Coordinator role, with a good understanding of person-centred care and care planning.
- Experience as an experienced Senior Care Worker who is ready to take the next step in their career and can demonstrate strong leadership potential, excellent organisational skills and a passion for delivering high-quality care.
- A confident communicator, able to build positive relationships with tenants, families and healthcare professionals, while remaining calm and organised in a busy environment.
- A proactive approach, good IT skills and a commitment to providing safe, compassionate care are essential.
