As part of our dynamic clinical and non-clinical team, you’ll help streamline care pathways, support long-term condition management, and ensure patients receive the highest standard of service. This is a role for someone who thrives on organisation, communication, and collaboration, and wants to contribute to a healthcare system that truly puts patients first.
You’ll be at the heart of our mission to provide efficient, effective, and compassionate care, working closely with GPs, nurses, and external agencies to coordinate support that improves lives.

Key Responsibilities
- Coordinating care for patients with complex health and social needs, ensuring they receive the right support at the right time.
- Building personalised care plans that make a real difference in patients’ lives.
- Acting as the link between GPs, nurses, and external agencies to keep care seamless and joined-up.
- Managing patient registers for frailty, dementia, learning disabilities, and palliative care.
- Organising and chairing clinical meetings, preparing agendas, and capturing key decisions in minutes.
- Handling referrals and urgent tasks with precision and prioritisation.
- Maintaining accurate patient records on EMIS to support safe, proactive care.
- Responding to patient queries and liaising with families and carers to provide reassurance and clarity.
- Driving improvements in care processes and contributing ideas to make services more efficient.
- Ensuring robust adult and child safeguarding practices are followed in every interaction.
