Are you a compassionate and experienced Registered Nurse looking for an opportunity to improve outcomes for some of our most vulnerable residents? NHS Greater Glasgow and Clyde is seeking a Care Home Liaison Nurse to join the East Dunbartonshire Health and Social Care Partnership Care Home Support Team. In this rewarding role, you will work collaboratively with care home staff, residents, families and multidisciplinary colleagues to deliver expert nursing support and promote high standards of person-centred care. You will play a key role in supporting care home residents to remain well, avoid unnecessary hospital admissions and experience smooth transitions between care settings. All roles will be assessed for sponsorship eligibility in line with current Home Office requirements. NHS Greater Glasgow and Clyde (NHSGGC) can only issue a Certificate of Sponsorship (CoS) where these are met.
Duration, Location, and Working Pattern:
- Part-time, Permanent
- You will be based in East Renfrewshire HSCP
- Working Pattern: 3 days over 7
Please note the salary is Pro Rata for part-time hours.
Key Responsibilities
- Provide specialist nursing advice, support and clinical leadership to care home staff caring for residents with complex needs
- Support hospital discharge planning and care transitions to ensure safe and effective transfers between care settings
- Help prevent unnecessary hospital admissions through proactive assessment, guidance and intervention
- Support care home staff to deliver high quality palliative, end of life and long-term condition management
- Undertake clinical assessments and provide specialist recommendations to meet residents' individual needs
- Deliver education, training and competency development to enhance the skills and confidence of care home staff
- Work collaboratively with multidisciplinary teams, primary care services and partner agencies to improve resident outcomes

Knowledge, training, qualifications and/or experience required to do the job:
- Registered Nurse with current NMC registration and relevant transferable clinical experience
- Experience working within community, acute, mental health or care home settings
- Educated to degree level or willing to work towards achieving a degree qualification
- Knowledge and understanding of caring for older people, chronic disease management and person-centred care
- Excellent communication, interpersonal and relationship-building skills
- Strong organisational, time management and IT skills with the ability to work independently and prioritise workload
- Evidence of continuous professional development and a willingness to undertake further learning in line with service requirements
