The association’s goal is to help maintain a sustainable choice of high-quality care services for local communities. LinCA supports providers by offering accessible information, advice, guidance, and development opportunities so they can adapt to changing needs with innovative, person-centered care. The organization works in partnership with a range of agencies and stakeholders to drive system-wide improvement in care provision. LinCA’s membership includes care homes offering personal and nursing care, domiciliary care services, community and day services, supported living providers, and independent social work professionals.
Role Description
The Care Home Trusted Assessor is a part-time, on-site role based in Kettering, responsible for assessing individuals for appropriate care home placements. The role involves conducting thorough assessments in hospital, reviewing care needs, and liaising with care homes to match individuals with suitable services. The Trusted Assessor collaborates with healthcare professionals, social workers, and families to ensure timely, safe transfers into care homes and to reduce delays in discharge. Daily tasks include completing assessment documentation, maintaining accurate records, providing clear communication to care providers, and monitoring outcomes to support quality and consistency of placements. The role also contributes to service improvement by identifying gaps, sharing feedback from providers, and supporting best practices in person-centered care.

- Strong assessment and care planning skills, with the ability to evaluate physical, emotional, and social care needs in a structured and objective manner.
- Experience in adult social care, nursing, allied health, or similar care settings, with a solid understanding of care home services and local care pathways.
- Excellent communication and interpersonal skills, including the ability to liaise effectively with individuals, families, care home staff, and multidisciplinary teams.
- Proficient in documentation, record-keeping, and basic IT systems to ensure accurate, timely, and confidential handling of information.
- Strong organizational and time-management abilities, with capacity to prioritize assessments and manage a varied caseload in a busy environment.
- Commitment to person-centered care, safeguarding, equality, and non-discriminatory practice, with a focus on dignity, respect, and informed choice.
- Relevant professional qualification or training in health, social care, or a related field is highly desirable; prior experience in care home or discharge planning roles is an advantage.
