Care Liaison Advisor
🌟 We’re Hiring: Care Liaison Advisor
Epsom Hospital (5 days per week on-site) 6-Month Fixed-Term Contract, Part-Time 22 hours per week Monday to Friday, 10:00am – 2:30pm, with weekend work on a rota basis £26,000 per annum (pro rata)
Do you thrive on building relationships and delivering exceptional customer care? We're looking for a Care Liaison Advisor to join our team and collaborate with hospital discharge teams and care homes to source the best care packages for our patients.

Our services help people avoid hospitals or accelerate their safe discharge, providing specialist support in their community or homes whenever needed.
đź’ˇ About the Role
In this role, you will:
- Source both short and long-term packages of care for patients, working towards a quick, safe and correct discharge plan.
- Effectively communicate with care homes, care agencies, healthcare professionals, and patients/families.
- Share updates with all relevant stakeholders to ensure a high-quality service delivery while meeting discharge timescales.
- Enter data onto the system and manage patient records.

📚 Experience We’re Looking For
- Strong administration and data entry experience (preferably with Microsoft Packages).
- Customer service experience, particularly handling high volumes of calls.
- Experience in fast-paced environments.
