🌟 We’re Hiring: Care Liaison Advisor
Epsom Hospital (5 days per week on-site)
6-Month Fixed-Term Contract, Part-Time 22 hours per week
Monday to Friday, 10:00am – 2:30pm, with weekend work on a rota basis
ÂŁ26,000 per annum (pro rata)
Do you thrive on building relationships and delivering exceptional customer care? We're looking for a Care Liaison Advisor to join our team and collaborate with hospital discharge teams and care homes to source the best care packages for our patients.
Part of Acacium Group, Xyla is an integrated healthcare solution that improves people’s lives and alleviates the demands of the UK healthcare system. Our services help people avoid hospital or accelerate them safely through it, with specialist support in their community or homes whenever needed.
đź’ˇ About the Role
In this role, you will:
- Source both short and long-term packages of care for patients, working towards a quick, safe and correct discharge plan.
- Effectively communicate with care homes, care agencies, healthcare professionals and the patients and their families.
- Share updates with all relevant stakeholders to ensure the successful delivery of a high-quality services whilst ensuring discharge happens within set timescales.
- Entering data onto the system and dealing with patient records.

📚 Experience We’re Looking For
- Strong administration and data entry experience, working with Microsoft Packages.
- Customer service experience, dealing with a large volume of calls.
- Experience working in a fast-paced environment.
