About the role
The Care Manager is responsible for delivering case management that is person centred and enablement focused, in accordance with Calvary Home Care policies and procedures, program, guidelines, and the National standards of practice for Care Management and the National Code of Ethics and Calvary Home Care Management Model.
Key Responsibilities
- Time Management and working within expected time frames
- Understanding the Aged Care Standards
- Liaising with stakeholders and internal networks
- Monitor and respond to changes in clients requirements with a person centred approach
- Utilization of innovative and problem solving skills
What you bring
- Demonstrated experience in the provision of Case Management services
- Demonstrated experience in the aged/community/disability sector particularly working with clients with complex needs
- Driver’s license
- Highly developed proficiency using computers including the Microsoft office suite with the ability to learn and adapt new databases
- Understanding of the financial principals of individualised client funding and experience in monitoring and reviewing individualised budgets
- An ability to work in a team and negotiate with internal and external stakeholders to ensure accountability for service delivery
- NDIS clearance (or willingness to obtain one)
- Police clearance (within the last 6 months or willingness to obtain one)
