Care Manager

Care Manager - Live-in Care

About the Role

At edyn.care, we’re redefining live-in care by combining exceptional carers, innovative technology, and outstanding operational support to help people live safely and independently in their homes.

As one of England’s highest-rated CQC-regulated live-in care providers, we’re growing our Quality Assurance team. We’re looking for a passionate Care Manager with a background in live-in care, domiciliary care, home care, or community care.

This hybrid role (London and the South East) requires expertise across care management, quality assurance, safeguarding, compliance, client assessments, care planning, and field-based support.

Key Responsibilities

  • Lead morning meetings with the care management and quality assurance teams to align on action plans
  • Conduct new client assessments and ensure smooth onboarding
  • Complete field and virtual visits, including spot checks, supervisions, and annual appraisals, to assess:
    • Professional carer wellbeing and performance
    • Client wellbeing
    • Care plan suitability
    • Environmental risk assessments
  • Collect quality assurance forms from clients’ homes and aid in auditing and feedback
  • Build and maintain strong relationships with clients and families, offering ongoing support
  • Collaborate with healthcare professionals (doctors, nurses, social workers) to ensure appropriate medical care
  • Ensure compliance with health and safety, safeguarding policies, and other regulations
  • Create and update outcome-based care plans as needed
  • Manage a two-week-advance field visit schedule
  • Report KPI data to senior management
  • Handle complaints and disciplinary processes per company policies
  • Cover out-of-hours rotations (every 5–6 weeks)

Requirements

  • 3–5 years’ experience in Health & Social Care
  • 2+ years as a Care Manager, Deputy Care Manager, Registered Care Manager, or Team Leader
  • Experience in live-in care, domiciliary care, home care, community care, or complex care
  • Deep knowledge of:
    • CQC standards
    • Safeguarding
    • Risk assessments
    • Compliance frameworks
  • Hands-on care assessments and person-centred care planning
  • Experience supervising and supporting care staff
  • Strong communication, relationship-building skills
  • Excellent organisation and case management abilities
  • Proficiency in digital care systems and Microsoft Office
  • Full UK driving licence and personal transport
  • Based within commuting distance of London
