Care Navigator – Fixed Term Contract (12 months)

Role Overview

Job Title: Care Navigator (Fixed Term Contract for 12 months) Salary: £31,750 per annum Hours: 37.5 hours per week Reports to: Operations Manager Key Relationships: General Practice, Healthcare LTD, One Westminster, Central and North West London NHS Foundation Trust (CNWL), Central London Community Healthcare NHS Trust (CLCH)


About Us

Central London Healthcare CiC is a GP Federation owned by 30 General Practices serving the Central London (Westminster) area. It supports 4 Primary Care Networks (PCNs), managing contracts for services including:

  • Community Dermatology
  • Community Cardiology
  • Secondary Care Referral Service
  • Out-of-hours provision (sub-contracted to General Practices)

As the host employer, we oversee a growing team of ARRS (Additional Roles Reimbursement Scheme) professionals such as: Clinical Pharmacists, Pharmacy Technicians, First Contact Physiotherapists, Dieticians, Paramedics, Social Prescribers, Care Coordinators, Digital & Transformation Leads, Nursing Associates, GPAs, and Care Navigators.

The Role

Core Purpose

A Care Navigator (CN) supports GPs in care coordination for vulnerable and complex patients, including:

  • Intelligent task management and patient referrals
  • Developing and updating Care Plans
  • Tailored public health messaging

CNs collaborate across healthcare, social care, and third-sector organisations to deliver patient-centred care, ensuring coordinated execution of Care Plans (identified through GP risk stratification). CNs are primarily non-patient-facing, supporting via telephony (Surgery Connect) and email, with occasional involvement in specialised case management.

This role is development-focused, involving structured planning and execution.

Key Responsibilities

Clinical Systems & Medical Records

  • Utilise SystmOne to record patient interactions per GDPR guidelines
  • Maintain accurate and up-to-date records
  • Respond promptly to assigned tasks
  • Support patients via telephony and email

General Duties & Responsibilities

  • Align Care Plans with personalised healthcare goals
  • Attend and lead MDTs (Multi-Disciplinary Teams) and ICTs (Individual Care Teams)
  • Coordinate health and care transitions across providers
  • Escalate unmet needs, raise safety concerns, and refer when necessary
  • Contribute additional administrative tasks as required
  • Meet team organisational targets collaboratively

Qualifications & Experience

Education & Skills

  • GCSEs (Grade C/4+ or equivalent)
  • Proficiency in written, verbal English, and numeracy
  • Keyboard skills for records documentation
  • Degree-level qualification in a relevant subject preferred

Technical Requirements

  • Experience with SystmOne
  • Knowledge of medical terminology
  • IT literacy essential

  • Broad knowledge of health/social care systems
  • NHS/Primary Care experience
  • Understanding of healthcare improvement methods
  • Flexibility, adaptability, and decisive judgement
