Outreach Support Worker - Make a Real Difference Every Day
Location: Central Sefton and surrounding areas
Hours: 20-40 per week (full or part time)
Contract: Permanent

About The Role
As an Outreach Support Worker, you’ll play a vital role in helping people live healthy, happy, and fulfilling lives. You’ll support individuals to maintain their independence, build confidence, and engage with their local community, whether that’s through daily routines, social activities, or developing life skills.
You’ll also assist with personal care, medication, and maintaining a safe, comfortable home environment. Every day will bring variety, purpose, and the opportunity to make a genuine difference.
We’re proud to offer flexible zero-hour contracts where you can choose to work as many hours as you’d like. To keep things fair for everyone, we ask for a minimum of 20 hours each week. We do ask for some weekend availability, but shifts are limited and balanced fairly across the team. You’ll work on a simple 2-week rolling rota, so you’ll always know your schedule in advance.

What You’ll Do
- Support individuals to live independently, including daily tasks like meal preparation, budgeting, and personal care.
- Encourage access to community activities, appointments, and social opportunities.
- Promote positive relationships and support emotional wellbeing.
- Administer and record medication accurately in line with policy.
- Help maintain a safe, clean, and welcoming home environment.
- Accompany individuals to appointments, hospital visits, or short breaks where required.
- Work closely with your Service Manager and team to follow and update care plans.
- Champion independence, dignity, and individuality in everything you do.
What You’ll Bring

- A commitment to treating every person with respect and empathy.
- Experience in a care or support role is helpful, but not essential; we’ll give you all the training you need.
