Care & Support Practitioner based at Ty Elai
Salary: £39,152 per annum - GR10
Rhondda Cynon Taf Council is pleased to offer an exciting Care & Support Practitioner position within the Care Management Team.
We want to ensure our services help people achieve their full potential. That means working with people to understand what matters to them and how we can help.
We are looking for a practitioner who is positive and enthusiastic and can work with individuals, families, and carers in line with the requirements and principles of the Social Services and Wellbeing Act (Wales) 2014, and the Mental Capacity Act 2005. We are seeking a practitioner who is able to put the person at the heart of their work.
Responsibilities:
- Undertake person-centred assessments
- Draw up care plans with individuals and their carers to ensure care needs are met
- Review care and support plans
- Work with individuals from the age of 18 years old who have care and support needs
- Undertake Section 126 enquiries in line with safeguarding and feed back outcomes
- Balance risk against self-determination and uphold the rights of the individual
- Deliver quality services

- Ability to appropriately manage risk
- Carries a varied caseload in a fast-paced environment, ensuring completion of assessments are met within timescales
- Excellent verbal and written skills and computer literate
- Organised, self-motivated, and able to prioritise competing demands on your time
- Creative, resourceful, and have a strong belief in empowering and enabling adults towards independence
- Professional development encouraged and expected in RCT
Desirable:
- Previous experience of working in a care management role or social care within Adult Services
- Knowledge and understanding of working within adult health / social care
- Conversant with all current legislation in Wales
