Case Manager
Multi-Trauma Case Manager – Remote – North Yorkshire
Location
Home-based with travel across North East and/or Yorkshire (2-hour radius of base location). Candidates must be based in [Newcastle, Durham, Sheffield, York, Leeds, Sunderland, Hartlepool, Harrogate, Doncaster, Darlington, Stockton-on-Tees, Bradford, Wakefield, and surrounding towns].
Hours & Salary
- 37.5 hours/week
- £39,000 – £45,000 per annum (dependent on experience)
About the Role
Compass Associates is partnering with a leading UK rehabilitation and case management provider to recruit Multi-Trauma Case Managers.
This is an exciting opportunity for experienced clinicians to support individuals following serious injuries, helping them achieve the best possible outcomes. You will work with clients experiencing complex orthopaedic trauma, internal injuries, musculoskeletal (MSK) conditions, and mild traumatic brain injuries.
Key responsibilities include:
- Conducting Immediate Needs Assessments (INAs) to assess clients' requirements.
- Proactively managing rehabilitation programmes in clients' homes and communities.
- Collaborating with clients, families, healthcare professionals, and stakeholders to develop person-centred rehabilitation plans.

Responsibilities
- Work directly with trauma survivors to design and implement personalised rehabilitation strategies.
- Undertake home and community assessments to understand individual needs.
- Coordinate care pathways, ensuring alignment with health, social care, and rehabilitation teams.
- Monitor progress and adapt plans to optimise client outcomes.
- Advocate for clients where required, liaising with hospitals, GPs, and specialists.
- Travel efficiently to client locations within the designated area.
Requirements
Essential:
- Qualified as a Nurse, Occupational Therapist, Physiotherapist, or Social Worker.
- HCPC, NMC, or Social Work England (SWE) registration in good standing.
- Minimum 2+ years of post-registration experience working with complex orthopaedic trauma, multi-trauma, MSK, and/or internal injuries.
- Full UK driving licence.
- Own personal vehicle (personal insurance required).
