Make a Difference
The Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring, and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of healthcare services. The Case Manager supports healthcare teams for a smooth transition between levels of care, while working primarily in NICU and potentially maternity when required.

Required:
- LCSW or LSW license in the State of Indiana
- Master’s Degree in Social Work
- 2+ years of experience in inpatient, outpatient, or home-based social work
Preferred:
- Experience with quality improvement methodology
- Alignment with Community Health Network’s vision and mission
Key Responsibilities:
- Regular collaboration with nursing staff and ancillary departments on discharge plans, barriers, or delays
- Participation in developing goal-directed plans of care, prioritized with intermediate goals and outcome criteria
- Coordination and facilitation of care in a knowledgeable, skillful, and consistent manner
- Accurate, timely, and clear documentation and patient record management for seamless continuity
- Upholding patient rights and sensitivity to significant others
- Sound decision-making for patient care with guidance when appropriate
- Active participation in staff development for care management teams and nursing personnel
- LCM discharge planning preparation and use of clinical pathways
- Self-directed learning and participation in continued professional development
- Awareness of legal aspects in patient care and unit function to minimize risk
- Contribution to care management team development and evaluation
- Effective communication within organizational hierarchies

Key Client-Centric Task Areas:
- Conduct discharge planning assessments, ensuring continuity of care
- Coordinate discharge plans with patients, physicians, and care teams
- Implement transition plans including referrals (home health agencies, ECFs transportation)
- Address unmet needs for safe and effective transitions
- Problem-solving to foster open communication with teams and staff
- Offer consulting skills for unit staff, care teams, patients, families, and physicians
- Comprehensive patient/family goal assessment, including:
- Biophysical, psychosocial, financial, and environmental factors
- Safe discharge planning needs
- Secure services and act as advocate for patients and families
- Provide liaison with post-hospital care providers and community resources
- Ensure compliance with Medicare, Medicaid, and third-party payer guidelines
- Complete all final disposition paperwork
- Conduct personal interviews, family conferences, and multidisciplinary meetings to ensure cohesive discharge plans
