Community Nurse Team Leader – Herne Bay (Open to NHS in Kent & Medway Employees)
Note: Currently open only to colleagues within Kent and Medway NHS Trusts, Kent and Medway ICB, Medway Community Healthcare OR Student/Recently Qualified Nurses.
About the Role
We are seeking a dedicated and adaptable Community Nurse Team Leader to join our innovative team in Herne Bay. This is an exciting opportunity to shape the future of community care in a diverse and supportive workforce.
Herne Bay serves a vibrant community with complex health needs, requiring a proactive and holistic approach to patient-centred care. As a Community Adult Nursing Team Leader, you’ll work closely with Primary Care Networks (PCNs) to deliver high-quality, needs-led services and support junior colleagues as they develop their clinical skills.
Key Responsibilities
- Lead a multidisciplinary team, managing patients with complex care needs in the community
- Take clinical leadership by autonomously planning and delivering progressive, tailored care
- Support junior nursing staff in their ongoing development, fostering a skilled and confident workforce
- Collaborate with local Primary Care Networks (PCNs) to refine innovative care strategies
- Build strong professional relationships across services to enhance patient outcomes
- Embrace ongoing clinical supervision, mentorship, and training to evolve as a strong leader

Essential Requirements
- Current NHS employment in Kent or Medway NHS Trusts, the ICB, or Medway Community Healthcare (or qualified/near-qualified nursing status)
- Specialist Practitioner (District Nursing) qualification (essential for post)
- Demonstrated experience in community nursing leadership
