CCT Health Care Support Worker
Job Overview
A new and exciting opportunity has arisen for a skilled part-time Health Care Support Worker (HCSW) to join the development of a redesigned Nursing Care Model with the Hillingdon Care Connection Team (CCT).
The CCT comprises:
- A Guided Care Matron
- A GP
- HCSWs
- An administrator
- Well-being and support assistants
This team provides active case management for adult patients with long-term conditions to prevent unnecessary hospital admissions.
Main Duties
⚠️ Community staff travel considerable distances throughout a typical day.
All applicants must:
- Have the ability to drive
- Have access to a car
Core Responsibilities (Clinical Support)
(Note: List not exhaustive)
- Conducting home visits for clinical tasks
- Joint visits with Matrons as required
- Conducting observations and using NEWS2
- Assisting with occasional medication monitoring (max. one week)
- Performing phlebotomy
- Caring for simple wounds
- Administering eye drops (short-term)
- Supporting urinalysis and inhalation
- Checking pressure areas
- Updating and maintaining documentation
Post-Induction Expectations:
- Work independently (in compliance with CNWL policies and guidelines)
- Escalate concerns in a timely manner to the Matron or GP, who have overall clinical responsibility

Job Title: Band 3 Health Care Support Worker
Job Summary
As a Band 3 HCSW, you will be a crucial contributor to patients’ experiences, closely working with individuals, families, friends, and caregivers.
Your key contributions include:
- Embracing a professional, kind, and responsive approach to service delivery.
- Ensuring best possible clinical outcomes by:
- Updating your skills and training per evidence-based policies
- Collaborating with Registered Nurses to review and implement nature plans.
- Empowering patients through respectful partnerships when maturing and evaluating care plans.
We value multi-disciplinary collaboration. Board Level HCSWs support the team by:
- Working closely with registered nurses and other health professionals.
- Facilitating seamless care across diverse teams and services.
Person Specification
Education & Qualifications
Essential criteria:
- Proficiency in numeracy and literacy to maintain accurate records and administer medicines (if applicable).
- Spoken and written English at a level necessary to perform job requirements.

Desirable criteria:
- NVQ Level 3 or equivalent
Experience
Essential criteria:
- Experience working with multi-disciplinary teams.
- Ability to communicate effectively with the general public, including diverse individuals.
- Experience in a fast-paced environment.
- Prior work in customer-focused roles.
Desirable:
- Healthcare/NHS experience in community or hospital settings.
- Volunteer sector involvement.
- Any background in caring professions.
Skills & Knowledge
Essential criteria:
- Copying skills for effective team communication, including face-to-face & telephone updates.
- Competency in recording accurate, up-to-date clinical information (both electronic and handwritten).
- Ability to discuss sensitive topics confidently while maintaining supportive or assertive engagement as needed.
Desirable:
- Basic knowledge of the Mental Health Act 1983.
Other Requirements
Must have:
- Readiness to undertake the Care Certificate and additional training as dictated by roles.
- Flexibility to work various shifts, including day and night duties.
