Charge Nurse

Crisis Team Mental Health Practitioner – CRHT/CRHTW Organisation: Hertfordshire Partnership University NHS Foundation Trust

About the Role

The Mental Health Practitioner (Crisis Resolution and Home Treatment) will support the Team Leader in delivering a multi-professional Crisis Resolution and Home Treatment (CRHT/CRHTW) service, ensuring high-quality care for service users and carers during acute mental health episodes.

The role involves providing leadership, supervision, and support to the multi-disciplinary team to ensure operational efficiency, team cohesion, and continuum care for individuals in crisis.

Key Responsibilities

Clinical Leadership and Care Delivery

  • Conduct mental health assessments for individuals in acute crisis.
  • Provide intensive treatment and support as an alternative to inpatient admission, aligning with clinical needs and service user preferences.
  • Undertake ward-based shift leadership in a crisis service/inpatient setting, coordinating care delivery.

Clinical Management and Supervision

  • Oversee mental health assessments, care planning, and evaluations while ensuring person-centered and evidence-based practice.
  • Take clinical leadership during shifts, making decision-making—especially in high-risk or crisis scenarios.
  • Provide guidance, supervision, and mentorship to junior staff, students, and support workers, promoting continual professional development.

Team Dynamics and Wellbeing

  • Facilitate effective team systems and processes, ensuring collaboration between multi-disciplinary staff.
  • Implement and monitor outcome measures and audit activities under the direction of the Team Leader.
  • Delegate tasks appropriately while ensuring safe staffing levels during shifts.
  • Foster staff wellbeing, promoting a supportive team environment aligned with the Trust’s Open Culture and Employee Charter.
