Fixed Term Secondment/Position: NHS Funded Registered Nursing Care Manager (Band 6 RGN) – Central Area (Conwy & Denbighshire)
Job Overview
This is a fixed-term/secondment position until 31 March 2027, funded through external resources.

An intriguing opportunity for a Band 6 Registered General Nurse (RGN) to join the Continuing Healthcare (CHC) team in the Central Area, covering Conwy and Denbighshire.
About the Role
A fixed-term position (until 31 March 2027) is available to manage NHS Funded Registered Nursing Care (FNC) and Continuing NHS Health Care (CHC) for patients in:
- Nursing homes
- Domiciliary settings
Your role is to oversee the implementation, monitoring, and review of FNC/CHC to ensure: ✔ A consistent, high-quality standard of care aligning with NHS best practice and funding guidelines ✔ Patients receive appropriate, evidenced-based support
Core Responsibilities
Care Governance & Implementation
- Lead the customary management of FNC and CHC within Conwy and Denbighshire as representative of Betsi Cadwaladr Health Board
- Maintain continuous compliance with national standards and NHS processes, updating as required

Assessment Coordination
- Direct and organise a Multi-Disciplinary Team (MDT) for CHC funding evaluations in:
- Acute and community hospitals
- Home-based settings
- Ensure assessments follow National Guidance and Local Standard Operating Procedures (SOPs)
Collaboration & Partnerships
- Work closely with local authorities and independent care providers to conduct joint reviews for FNC/CHC patients
- Provide nursing guidance to care staff regarding appropriate patient referrals across clinical teams
Training & Education
- Develop and deliver training programmes on CHC and FNC funding standards
Supporting Equality & Multilingualism
- Welsh-language proficiency is desirable but not mandatory. Applications in either English or Welsh are equally welcomed.
