About the Role
A clinical leadership opportunity for a Registered Children’s Nurse with experience managing complex care packages—are you ready to step into this challenging and rewarding role?
We’re recruiting a Children’s Nurse Case Manager to join Routes Healthcare’s specialist complex care team on a full-time, permanent basis. You’ll hold clinical responsibility for a caseload of children with complex health needs, delivering assessments, overseeing care delivery, and providing essential support to families, frontline staff, and healthcare professionals.
This is a community-based, travel role covering the North of England, with a preference for candidates based along the M62 corridor.
Responsibilities
- Take clinical ownership of a diverse caseload of children with complex needs, ensuring high-quality, holistic care
- Lead assessments, care plans and risk reviews to support safe, personalised care delivery
- Provide clinical supervision and competency sign-off for direct care staff
- Act as the primary clinical point of contact for families, commissioners, general practitioners (GPs), consultants, community nursing teams and other healthcare professionals
- Support the set-up of new care packages, including initial assessments and staff training
- Manage clinical escalations and support incident reviews for timely intervention
- Contribute to audits, quality assurance and CQC compliance processes

Key Clinical Interventions (requires working knowledge)
- Tracheostomy care and management
- Ventilation (invasive and non-invasive)

Requirements
✅ Must be a Registered Children’s Nurse (RCN) with a valid NMC Pin ✅ ** esencial proven experience** in trachestomy and ventilation care ✅ Highly desirable: Previous case management, community nursing, or clinical leadership experience ✅ A confident communicator able to engage effectively with families, clinical teams, and commissioners ✅ Organised, accountable, and able to manage varied caseloads independently
