General Summary
The Clinical Chart Review supports the patient’s care provider by coordinating maintenance healthcare to optimise health across conditions such as diabetes, hypertension, hyperlipidemia, and more.
Key responsibilities include:
- Initiating care and orders for assigned patients via a predetermined algorithm.
- Ensuring desired health outcomes through structured processes.
- Managing evaluation and follow-ups for all scheduled patients across assigned offices.
- Maintaining open communication with providers and healthcare staff to guarantee patient-centred care.
Duties & Responsibilities
- Act as a valued member of the clinical team, delivering patient-centred care.
- Review and flag scheduled patient charts via electronic alerts to address:
- Urgent care concerns for the provider’s next visit.
- Clinical follow-up needs (e.g., diagnostics, treatments).
- Order diagnostic testing as needed under protocols defined by nursing and provider leadership.
- Apply adaptive algorithms for:
- Fall Risk Assessment
- Smoking Cessation support
- BMI (Body Mass Index) monitoring
- Mammogram and Bone Density screenings
- Pneumovax (Pneumococcal vaccine) reminders
- Fecal Occult Testing
- Diabetic health measures
- Collaborate with providers and staff to assist with additional office tasks.
- Perform other duties as assigned.

Requirements
Education & Experience
- Minimum: High School Diploma or equivalent.
- Licensure: LPN or state-authorised Medical Assistant (MA) certification.
- 3–5 years of nursing experience, with proficiency in Electronic Medical Records (EMR) systems.

- Independent workflow adherence with accountability to health protocols.
- Highly detail-oriented regarding patient records.
- Strong written and verbal communication skills.
- Excellent time management, organisation, and problem-solving abilities.

Essential Physical & Mental Demands
- Communication: Active listening and precise spoken/written exchanges.
- Observation: Accurate perceptions of patient conditions, workload accuracy, and workplace observations.
- Physical Stamina:
- Steady upright posture (75% of the time).
- Mobility via walking, bending, stretching, and manual handling of objects.
- Cognitive demands:
- Task flexibility to adapt to interruptions/disruptions.
- Decision-making under standard office policy guidelines.
- Quick prioritisation adjustments.
Marginal Physical Challenges
- Extended Stationary Work: At least 25% of shift in fixed positions.
- Manual Handling: Occasional lifting/moving objects weighing up to 50 pounds.
- Perception: Tactile recognition (e.g., distinguishing temperature/texture) and colour differentiation.
- Innovation: Identifying efficiency improvements in workflow.
