Clinical Lead - Yorkshire
We’re Hiring: Clinical Lead Permanent, Full Time + on call telephone support on a rota system Field based with frequent travel across Yorkshire £49,000-£53,000 + £3,000 car allowance Are you passionate about delivering exceptional care and making a real difference in the community? We’re looking for a Clinical Lead to join our team and manage a caseload of adult and paediatric packages across Yorkshire. About the Role In this role, you will: Undertake initial holistic nursing assessments in partnership with the client, their family and the care team. Develop individualised care plans in partnership with the client, their family and other healthcare professionals. Effectively communicate with staff on the clinical condition/diagnosis and treatment/discharge plans of clients and ensure accurate records are maintained by staff. Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation. Support staff by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.
