Role Overview
The role requires a highly skilled Nurse who will be able to clinically lead the Hospital Liaison Team across a 24/7 rota to support the Team to provide a response to mental health crisis in Emergency Departments and Inpatient Wards within one hour and to all urgent Ward referrals within 24 hours.
Key Responsibilities
- Provide specialist clinical advice, support, and supervision to staff at Scunthorpe General Hospital and to junior staff within the Hospital Liaison Team.
- Actively involve in change processes and support junior staff through this process.
- Operate under the Lone Working Policy.
- Undertake comprehensive mental health, psychosocial, and risk assessments for patients presenting to the Emergency Department and inpatient wards with complex mental health needs.
- Provide timely specialist mental health interventions, treatment planning, and risk management, including assessment of patients presenting with suicidal thoughts or behaviours.
- Support delivery of national response standards and crisis care pathways, ensuring prompt assessment and appropriate follow-up care.
- Provide clinical leadership, specialist advice, and support to Liaison Mental Health Team staff, acute hospital colleagues, and partner agencies.
- Coordinate care, discharge planning, and onward referrals, working closely with multidisciplinary teams, patients, families, and carers.
- Manage a complex caseload and contribute to the allocation and prioritisation of referrals to ensure safe and effective service delivery.
- Promote high standards of clinical practice, governance, safeguarding, and compliance with relevant legislation, including the Mental Health Act and Mental Capacity Act.
- Contribute to service development, quality improvement, audit, training, and education programmes for both mental health and acute hospital staff.
- Support staff development through supervision, mentorship, coaching, and clinical training.
- Deputise for the Team Leader when required, contributing to the day-to-day operational management of the service.
