Clinical Nurse Specialist for Palliative Care

We are looking for a well-motivated Clinical Nurse Specialist for Palliative Care with excellent communication, organisation and leadership skills. This is a maternity post starting in October 2026 and running until June 2027.

Cheshire Wirral Partnership NHS Trust are looking to appoint a Clinical Nurse Specialist for Palliative Care to work alongside community colleagues within the Neighbourhood Community Care Team based in the Fountains Health Centre, Chester.

About the Role

As a Clinical Nurse Specialist for Palliative Care, you will be supported by fellow specialist practitioners including other Nurse Specialists, Occupational Therapist, Physiotherapists and a Therapy Assistant. You will also clinically work closely with the support of the Palliative Care Consultants.

The successful candidate will receive a full supportive induction when they join. You will work closely with both your neighbourhood team, Specialist Palliative Colleagues and wider Multi-disciplinary colleagues based at the Hospice and beyond.

Responsibilities

  • Provide holistic care and support to patients with complex palliative care needs with malignant or non-malignant life limiting conditions, working autonomously and as part of a team.
  • Deliver care to patients registered with a West Cheshire GP in their own home.
  • Provide advice, education and support to patients, their carers and other health care professionals.
  • Promote advanced care planning, management of complex end of life symptoms and patient and carer expectations whilst meeting the holistic care needs of the patient.

  • Experience working in the community, supporting people in their own homes.
  • Experience in Palliative Care.

Additional Information

  • This role is part time, 30 hours per week.
  • All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role.
