Job Summary

30 hours per week - 12 months fixed term

A fantastic opportunity has arisen for a motivated and enthusiastic pharmacist to join Southwest Yorkshire Partnership NHS Foundation Trust for a fixed term. We welcome applications from qualified pharmacists who are looking for a change to develop in another setting or those who already work in the environment and would like to progress further.

The post is based at Fieldhead Hospital, Wakefield with occasional travel required to other trust localities. It offers an excellent role for an experienced pharmacist to progress and experience the role in a mental health setting.

The successful applicant will join a supportive, friendly and forwardthinking team. You will be encouraged to grow your confidence and clinical skills, apply your learning in practice, and develop into the pharmacy professional you aspire to be.

While this role meets the eligibility requirements for Skilled Worker sponsorship, due to the short-term nature of the contract, the Trust will not be offering sponsorship for this post.

Main duties of the job

The post holder will provide pharmacy services across a range of mental health settings, including inpatient wards, forensic units, community mental health teams and learning disability services. You will also work closely with primary care and wider healthcare teams through medicines information, advice and guidance to both professionals and patients.

The Role Will Involve:

  • Provision of clinical pharmacy services to allocated teams.
  • Delivery of high-quality, patient-focused pharmaceutical care.
  • Providing medicines information and advice to healthcare professionals and patients.
  • Active contribution to multidisciplinary teams, where pharmacy input is valued and supported.
  • Opportunities to participate in clinical audit and quality improvement.
  • Contributing to pharmacy team meetings and service development.
  • Participation in the Saturday, Bank Holiday and on-call rotas.

JOB SUMMARY

JOB PURPOSE

To participate in the provision of a safe and effective specialist clinical pharmacy service to designated wards within SWYPFT. To assist Principal Pharmacist / Senior Clinical Pharmacist in the overall provision of pharmacy services to units/teams within SWYPFT. To deputise for Principal Pharmacist / Senior Clinical Pharmacist as appropriate.

ROLE OF YOUR DEPARTMENT

  • Maintain and develop efficient and cost-effective systems for procurement, compounding, preparation and distribution of medicinal products, which reflect the needs of individual patients/patient groups as determined by the Trust.
  • To ensure compliance with the current law relating to medicines and medicinal products.
  • To consider and adopt as appropriate Department of Health advice and guidance relating to the pharmaceutical services provided.
  • To provide professional advice to relevant healthcare professionals and patients regarding the safe, cost-effective use of medicines and medical products.
  • To identify and ensure the operation of quality control mechanisms that meet formal controls required by the medicines agencies.
  • Identify and address clinical risk issues by applying principles of clinical governance, Medicines Management and controls assurance.

RESPONSIBILITIES (Budgetary/Supervisory)

Supervisory to liaise with the Principal Pharmacist / Senior Clinical Pharmacist in coordinating the activities of:

  • Clinical supervision: Band 6 (Pharmacists)
  • Band 5 (Technicians)

Coordinate activities of: Ward Based Technician(s), Assistant(s)

This job description is an outline of the main duties of the post. The postholder will be required to undertake other duties commensurate with the grade as directed.

The content of this post will be reviewed in consultation with the postholder when necessary and in line with the service developments.

KEY RESULT AREAS:

To provide a specialist clinical service to wards and departments of SWYPFT in accordance with Trust and department policies. This will include:

  • Taking drug histories on admission.
  • Reviewing medication to ensure safe and effective use.
  • The formulation of individual care plans.
  • Reviewing and updating care plans depending on response, results of investigations and adverse effects.
  • Therapeutic drug monitoring for specific drugs.
  • Educate patients about their drug therapy.
  • Develop evidence-based treatment protocols for use either within the directorate or Trustwide where appropriate and work with other pharmacy colleagues to comment on and support implementation of policies and procedures.
  • Facilitation of compliance aids and self-medication schemes where appropriate in order to increase patient safety, improve concordance and increase quality of life in this unique patient group.
  • To undertake clinical audit and practice research within the Pharmacy Department where appropriate.
  • To record clinical interventions and complete query record forms in line with departmental policy. To collate all Trust pharmacist clinical interventions and report on trends and outcomes where requested as appropriate by the Principal Pharmacist / Senior Clinical Pharmacist.
  • Promote evidence-based cost-effective prescribing.
  • To provide professional and legal clinical supervision in the dispensary and to participate in the checking and dispensing of prescriptions (including compliance aids) when required as directed by the Technical Services Manager.
  • To develop and maintain links with other professionals, such as dieticians, nursing and medical colleagues regarding service developments and improvements. To recommend service changes and implement them where appropriate. This may include changing paperwork, changing storage or delivery arrangements etc.
  • To support nursing staff and Medicines Management pharmacy technician by providing clinical pharmacy advice to the patients self-administration scheme and to undertake ongoing evaluations of individual outcomes of patient self-administration.
  • To participate in the appropriate reporting process: to report, record and monitor medicines-related incidents or complaints and to explore ways of minimizing risk of future re-occurrence or risk to staff.
  • To undertake the dispensing of clinical trials in accordance with the dispensing protocol, ICH GCP guidelines and European Directives.
  • To provide clinical training for Pharmacy Undergraduates, Pre-registration Pharmacists, Postgraduate Pharmacists and Technicians.
  • To participate in the training of medical and nursing staff as required.
  • To participate in the out-of-hours rotas.
  • To comply with the legal and other requirements related to the purchase, supply, use, safe custody and destruction of drugs within Pharmacy and in all other areas in the Trust.
  • To comply with the Health and Safety at Work Act, Data Protection Act and COSHH legislations.
  • To participate in Continuing Professional Development as per department policy and national guidelines from the General Pharmaceutical Council.
  • To participate in individual performance review.
  • To assist in the development of a Medicines Management strategy and related policies by attending the Pharmacy Communication Group and other Trust groups as directed by the Principal Pharmacist / Senior Clinical Pharmacist.
  • To contribute to the development and monitoring of pharmacy objectives set by the national/regional and local agencies and others to meet clinical governance targets, develop patient care, improve clinical outcomes and ensure compliance with all standards expected for hospital pharmacy services and the interface between primary and secondary care.
  • To ensure adherence to local and national guidance and formularies which exist e.g. Hospital formulary, NICE guidelines, Area Prescribing Committee etc.
  • To provide clinical and ward pharmacy services within the Trust as required in respect of absence or leave arrangements for other Trust Principal Pharmacists.
  • Following qualification as an independent non-medical prescriber, prescribe medication as an independent prescriber within their own sphere of competence and the bounds of their approved prescribing status in accordance with the Trust non-medical prescribing policy.
  • Promote patient choice and understanding when communicating assessment outcomes or prescribing medicines, including provision of high-quality information both written and verbal to support this.
