Clinical Pharmacist - Circle Health Group
Clinical Pharmacist – Permanent (22.5 Hours/Week)
About the Role
Woodlands Hospital, based in Darlington and part of Circle Health Group—Britain’s leading independent healthcare provider—seeks a dedicated Clinical Pharmacist to join their Pharmacy Department.
This is a permanent, part-time role (22.5 hours/week) with a flexible shift pattern. Salary is up to £48,300 per year, depending on experience, training, and qualifications.
Key Duties & Responsibilities
Medicines Management & Patient Care
- Conduct medication reviews for inpatients and outpatients, identifying:
- Drug interactions
- Contraindications
- Opportunities for medicines optimisation
- Provide personalised patient advice on:
- Treatment plans
- Side effects
- Compliance (adherence)
- Lifestyle considerations
Safety & Prescribing
- Support safe prescribing through:
- Prescription reconciliation (ensuring accuracy)
- Clinical screening (risk assessment)
- Dose adjustments
- Authorise prescriptions in line with:
- Clinical guidelines
- Formulary restrictions
- Regulatory requirements (e.g., MHRA, NHS policies)

Patient & Cost-Effectiveness
- Promote safe and cost-effective medicine use by adhering to local/national policies.
- Collaborate with:
- Procurement teams
- Stock control
- Pharmacy technicians to ensure medicine availability and supply continuity.
Risk Monitoring & Incident Reporting
- Monitor and report:
- Medication incidents
- Near misses
- Adverse drug reactions in compliance with hospital protocols and MHRA guidance.
Requirements & Qualifications
Essential Criteria
- Degree in pharmacy (MPharm, BSc, or BPharm).
- GPhC registration.
- Experience in a clinical hospital pharmacy role.
- In-depth knowledge of:
- Therapeutics
- Clinical guidelines
- Pharmaceutical legislation
- Strong communication and clinical decision-making skills.
- Ability to:
- Work autonomously or as part of a multidisciplinary team.
- Proficiency in electronic prescribing and EPMA systems.
