Clinical Pharmacist (Development Role 7-8a)
About the Opportunity
We have an exciting opportunity for clinical pharmacists to join our pharmacy team, working across community pharmacy and frailty services. This development role would suit either newly qualified pharmacists or more experienced pharmacists who meet the higher-level criteria in the person specification.
Responsibilities

Split your working week between acting as the responsible pharmacist in our community pharmacies and delivering frailty services within CHCP.

Provide comprehensive geriatric assessment to patients in:
- The Jean Bishop Integrated Care Centre
- Care homes
- Our community bedded units
- Our hospital at home frailty virtual ward (including home visits)

Play an active role in delivering NHS services, including:
- Pharmacy First
- Vaccinations
- NMS and DMS
Requirements
- Motivated pharmacist with strong clinical skills.
- Effective communication with customers, patients, GPs, and other healthcare professionals.
- Ability to travel across the areas where our services operate.
- Flexible to meet organisational needs.
