Job Description:

Clinical Pharmacist Independent Prescriber | Locum | Swindon | Up to Full Time | Hybrid Working

An excellent opportunity has become available for an experienced Clinical Pharmacist Independent Prescriber to join a friendly, forward-thinking GP practice in the Swindon area on a locum basis.

This is an immediate requirement with an initial 3-month contract and the potential for extension or joining them on a salaried basis (if you wanted to, not obligation to do so). The practice can offer a flexible hybrid working arrangement with up to 2 days working remotely each week.

Location: Swindon

Start Date: ASAP

Contract: 3 Months Initially

Hours: Minimum 4 days per week, full-time preferred

Working Pattern: Up to 2/3 days on site and 1/2 days remote

Clinical System: SystmOne

Hourly Rate: Competitive and dependent on experience

The Role

You will become an integral member of the practice's multidisciplinary team, supporting the safe and effective management of patients' medicines whilst helping to reduce GP workload.

Responsibilities include:

  • Face to face Structured Medication Reviews (SMRs)
  • Face to face medication reviews
  • Medication reconciliation following hospital discharge
  • Supporting Pharmacy Technicians with clinical queries
  • Managing prescription queries through Repeat Prescription Authorisers (RPA) and task groups
  • Supporting the pharmacy team with general clinical tasks
  • Reviewing and actioning complex discharge summaries and clinic letters
  • Working collaboratively with GPs, nursing staff, and the wider multidisciplinary team

The Ideal Candidate

  • Registered Pharmacist with the GPhC
  • Independent Prescriber qualification
  • Previous experience working within UK General Practice
  • Confident using SystmOne
  • Strong knowledge of Primary Care prescribing and medicines optimisation
  • Able to work independently whilst contributing positively to the wider practice team
  • Excellent communication and organisational skills
