Are you a passionate Clinical Pharmacist Independent Prescriber looking to make a real difference in primary care?
At Bourne Health Partnership, we're looking for a proactive and patient-focused Clinical Pharmacist to join our friendly and supportive team at Thornton Road Surgery. This is an exciting opportunity to work alongside experienced GPs, nurses, and our wider pharmacy team, helping to deliver high-quality, personalised care to our patients.
What you'll be doing:
- Managing long-term conditions and carrying out structured medication reviews
- Prescribing and optimising medicines for patients with complex needs
- Supporting GPs and the wider clinical team with expert medicines advice
- Reviewing hospital discharge letters and medication reconciliations
- Helping shape safer prescribing through clinical audits and quality improvement

We'd love to hear from you if you have:
- Independent Prescribing (IP) qualification
- GPhC registration
- Experience in primary or hospital pharmacy
- A passion for improving patient outcomes and collaborative working
