Clinical Pharmacist Opportunity
Due to expansion and career progression within our team, we have an exciting opportunity for a Clinical Pharmacist to join our organisation.
We are looking for an enthusiastic and dynamic pharmacist to work within Nottingham City South PCN.
Desirable Qualifications
- Completion of CPPE pathway and independent prescribing are desirable but not essential.
Desirable Experience
- 2 years experience of pharmacy work within the healthcare setting or equivalent is desirable but not essential.
Responsibilities
The post holder will work as part of a multi-disciplinary team in a partially patient-facing role. The post holder will be responsible for areas of Chronic disease management, clinical medication reviews, supporting the general practice staff with regards to prescription and medication queries.

- Provide support for the repeat prescription system
- Deal with accurate prescription requests
- Conduct medicine reconciliation on transfer of care
- Implement systems for safer prescribing
- Provide expertise in clinical medicine advice
- Provide clinical leadership on medicines optimisation and quality improvement
- Manage some aspects of the quality and outcomes framework and enhanced services
