This is an exciting opportunity to join our team as a Clinical Reconnect Practitioner in the Humberside region.
Hours
- Monday – Friday: 9am - 5pm
Salary

Key Responsibilities
As a Clinical Reconnect Practitioner, you will:
- Operate autonomously in prison environments, detention centres, and the community.
- Identify and respond to cases requiring health screening and assessment of individuals with urgent health needs—particularly those leaving custody with less than 12 weeks remaining on sentence or within 28 days of release.
- Provide initial referrals to ensure continuity of care, reducing reoffending by addressing both health and social needs.
- Use validated tools to assess complex health needs (physical, mental, social), ensuring informed decision-making. This includes crafting individualised support plans and writing detailed, standard-compliant reports.
- Facilitate diversion from reoffending by linking individuals with appropriate intervention services.

What We Look For in You
Qualifications and Experience:
- Qualified professional with current registration from a relevant body:
- Registered Nurse (RN)
- Mental Health Nurse (MHN)
- Social Worker (SW)
- Occupational Therapist
- Demonstrates ongoing professional development and post-registration training.
- Proven experience in face-to-face support for patients/clients with complex mental health needs.
- Knowledge of psychosocial interventions and pathways for referral.
- Awareness of available substance misuse support services.
