Role Description

As a clinical specialist midwife in a continuity team you will provide care to a defined caseload of patients within a team continuity model, as described by Better Births (2016) in line with the ambition to reduce maternal and neonatal deaths, stillbirths and hypoxic-ischaemic encephalopathy.

In line with the Equity and Equality Strategy (2021) you will provide safe maternity care for those living in the most deprived areas to achieve health outcomes that are as good as the groups with the best health outcomes. This will be piloted within the Skegness Continuity of Carer Team and rolled out to other areas where need is identified.

You will work in the capacity of a clinical specialist in a Continuity of Carer team, for a caseload high in safeguarding and deprivation.

As part of a continuity team, you will cover an on-call rota and provide intrapartum support for patients booked for care with your named team.

Lincolnshire Community Health Services NHS Trust (LCHS) and United Lincolnshire Hospitals NHS Trust (ULHT) have come together in a Group arrangement, with the goal of improving the care provided to patients across the county.

This will not constitute a formal merger of the two organisations, but will bring the Trusts together under a single Board and Executive Leadership Team, with the goal of improving the care that is provided to patients both in the community and in hospitals across Lincolnshire. Both Trusts will retain their separate statutory names and legal obligations.

The Group will be known as Lincolnshire Community and Hospitals NHS Group (LCHG).
