Exciting Opportunity for a Band 6 Nurse

An exciting opportunity has arisen for a band 6 nurse to join the diabetic specialist nursing team. These specialist nurses are based throughout the trust however provided specialist advice for all specialities within the Trust.

Responsibilities

The successful candidate will be required to undertake clinical assessments for our patients and liaise with the wider MDT regarding ongoing care and treatment plans. We strive to deliver a high standard of care to our patients ensuring that safe and timely care is provided at all times enabling both the patients and the the MDT to manage the acute / chronic conditions in the appropriate environment.

The Nurse Specialist will provide support for the patient and family from diagnosis through treatment and throughout the diabetes journey. Using advanced communication and psychological assessment skills the CNS will aim to assess and provide management and education for diabetes patients and their families, including identification of need for onward referral to other agencies and disciplines as appropriate.

Requirements

We are looking for enthusiastic and motivated individuals who have an interest in diabetes. Successful candidates must be a minimum of 2 years qualified or have diabetes experience.

Training and Support

We will provide specialist training as well as the support from the MDT team.
