Responsibilities

Locala is inviting applications from Healthcare Assistants who share our commitment to person-centred care and want to make a real difference to patients’ lives.
Role
As part of our Community Nursing Network, you’ll work alongside qualified nurses to provide holistic, seamless care for housebound patients. Your role will include:
- Supporting nurses with assessments and care planning
- Delivering nursing-based care to prevent unnecessary hospital admissions
- Promoting self-care for patients with long-term conditions
- Contributing to health promotion and innovative service improvement projects
We have nine teams across Kirklees, offering flexibility to support work-life balance. Our service operates between 8:00 am and 9:00 pm, with core hours from 8:00 am to 5:00 pm. You’ll be expected to work one weekend in four and two late shifts (1:00 pm - 9:00 pm) over a four-week period. Car driver essential.
