About the Role
The post holder will provide leadership as a Specialist Community Co-Occurring Practitioner, working closely with both Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS) in local County-based services.
Key Responsibilities
We are seeking a motivated and experienced practitioner for the Specialist Community Co-Occurring Practitioner role to provide a service across the Denbighshire Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS). This key role will:
- Support the development and delivery of a local co-occurring pathway, improving outcomes for individuals experiencing both mental health and substance use needs.
- Lead the implementation and monitoring of the Co-Occurring Framework.
- Maintain a clinical caseload while ensuring high standards of care within a robust governance framework.
Working collaboratively, you will:
- Build strong integrated relationships with multidisciplinary teams, statutory partners, and third-sector organisations.
- Conduct comprehensive assessments and develop effective care plans.
- Contribute to service improvement through audit and evaluation.
- Deliver training to enhance clinical practice across services.

Person Specification
Essential Qualifications and Experience
- Qualified Health Professional with:
- An RMN qualification.
- A Professional degree.
- Demonstrable significant personal and clinical development.
- Management experience with evidence of autonomous leadership.
- Progress to an MSc or completion of MSc-level qualification within an agreed timeframe.

- Community experience in mental health settings.
- Supervisory experience.
- Advanced knowledge of community mental health team operations, including:
- Co-occurring mental health and substance misuse issues.
Attitude and Abilities (Essential Criteria)
- Communication skills (verbal, written, and IT proficiency).
- Leadership qualities (ability to steer teams through change processes).
- Clinical and management leadership skills.
- Strong risk assessment competence.
- Comprehensive knowledge of:
- Co-occurring Mental Health and Substance Misuse.
- Research and evidence-based interventions in these areas.
- Mental Health Measures (legislative alignment).
- Current Health & Social Care guidelines.
- Pioneering expertise in clinical governance strategy within the Co-Occurring Framework, spanning CMHT and SMS.
Desirable Criteria
- Welsh language fluency (for local engagement).
- Expertise in care planning and problem-solving.
- Experience in service user and carer involvement.
- Understanding of recovery and rehabilitation principles.
