Specialist Community Co-Occurring Practitioner
Job Overview
The post holder will provide leadership as a Specialist Community Co-Occurring Practitioner, working closely with both Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS) in local County-based services.
We are seeking a motivated and experienced practitioner for the Specialist Community Co-Occurring Practitioner role to provide a service across Denbighshire CMHT and Substance Misuse Services (SMS).
This key role will support the development and delivery of a local co-occurring pathway, improving outcomes for individuals experiencing both mental health and substance use needs.
You will:
- Lead the implementation and monitoring of the Co-Occurring Framework
- Maintain a clinical caseload while ensuring high standards of care within a robust governance framework
- Work collaboratively with multidisciplinary teams, statutory partners, and third-sector organisations
- Build strong integrated relationships to support service users and their families
- Conduct comprehensive assessments and develop effective care plans
- Contribute to service improvement through audit and evaluation
- Deliver training to enhance clinical practice across services
This is an excellent opportunity for a skilled practitioner with strong leadership abilities and a passion for integrated, person-centred care. The ability to speak Welsh is desirable, though English- and/or Welsh-speaking applicants are equally welcome to apply.

Person Specification
Essential Criteria
Qualifications and Knowledge
- Hold qualified health professional status (e.g., RMN qualification)
- Professional degree evidence
- Significant personal and clinical development experience
- Demonstrated management experience and ability to work autonomously
- MSc qualification in an agreed area, or pursuing/working towards completion within an agreed timescale
- Community experience
- Supervisory experience
- Sound understanding of community mental health teamwork, including advanced knowledge of mental health with co-occurring substance misuse issues

Essential Criteria
- Excellent communication skills, including IT proficiency
- Ability to lead staff and manage processes of change
- Strong risk assessment capabilities
- Clinical and management leadership skills
- Extensive knowledge of co-occurring mental health and substance misuse issues
- Understanding of research and evidence-based interventions in relation to co-occurring mental health and substance misuse
- Knowledge and application of the Mental Health Measure
- Knowledge of current legislation and the Health & Social Care agenda
- Ability to lead the clinical governance strategy within the Co-Occurring Framework pathway, integrating CMHT and SMS teams
Desirable Criteria
- Ability to speak Welsh
- Good care planning and problem-solving skills
- Experience in service user and carer involvement
- Application of Recovery and Rehabilitation principles
