Job Overview

The post holder will provide leadership as a Specialist Community Co-Occurring Practitioner working closely with both Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS) in local County based services.

Main duties of the job

We are seeking a motivated and experienced practitioner for the Specialist Community Co-Occurring Practitioner to provide a service across the Denbighshire Community Mental Health Teams (CMHT) and Substance Misuse Services (SMS). This key role will support the development and delivery of a local co-occurring pathway, improving outcomes for individuals experiencing both mental health and substance use needs.

  • You will lead the implementation and monitoring of the Co-Occurring Framework, while maintaining a clinical caseload and ensuring high standards of care within a robust governance framework.
  • Working collaboratively with multidisciplinary teams, statutory partners, and third-sector organisations, you will help build strong integrated relationships to support service users and their families.
  • The role includes conducting comprehensive assessments, developing effective care plans, contributing to service improvement through audit and evaluation, and delivering training to enhance clinical practice across services.

This is an excellent opportunity for a skilled practitioner with strong leadership abilities and a passion for integrated, person-centred care.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

  • Qualified Health Professional with an RMN qualification
  • Professional degree
  • Evidence of significant personal and clinical development
  • Management experience and evidence of being able to work autonomously
  • MSc or working towards completion of an MSc level qualification in an agreed timescale
  • Community experience
  • Supervisory experience
  • Sound understanding of community mental health team work to include advanced knowledge of Mental Health with co-occurring substance misuse issues

  • Knowledge and application of the Mental Health Measure
  • Knowledge of current legislation
  • Understanding of the Health & Social Care agenda
  • Ability to lead the clinical governance strategy within the Co-Occurring framework pathway across the CMHT and SMS teams
