Nurse – Recovery Focused Care Co-ordinator (Substance Misuse)
Job Purpose
To co-ordinate recovery-focused care packages for a defined caseload of clients with substance misuse issues, covering those in core services, satellite clinics, or primary care. Clients will have multiple complex needs, requiring multidisciplinary team and external professional network collaboration.
The role includes:
- Delivering physical health, harm reduction, psychosocial, and recovery interventions
- Developing individualised recovery plans based on comprehensive assessments (physical, psychological, mental health, forensic, and social)
- Clinical lead for caseloads with complex drug/alcohol-related problems
- Specialist oversight of allocated areas such as medication management, BBVs (Blood Borne Viruses), rough sleepers, women’s health, and community alcohol detoxes.
- Research involvement, audit, and service development
- Daily operational support to ensure efficient and inclusive client care
Key Responsibilities
Clinical Care & Recovery Coordination
- Conduct comprehensive individual assessments (physical, psychological, mental health, forensic, and social needs) to inform personalised recovery plans.
- Implement and monitor SMART (Specific, Measurable, Achievable, Relevant, Time-bound) recovery goals regularly with clients.
- Ensure accurate and timely clinical documentation in systems such as NTDMS, adherence to trust data requirements.

Direct Client Interventions
- Provide physical health and harm reduction (e.g., medication management, vaccinations, injections, testing, health screening).
- Develop advances practice in specialist areas and mentor/judge recovery workers and students.
Organisation & Governance
- Develop and monitor clinical standards, audit practices, and research contributions.
- Support the daily running of the team, escalating concerns to senior staff as needed.
- Advocate for clients across statutory and voluntary organisations, fostering positive relationships.
Team & Service Development
- Engage in new practice, policy, and guideline development to enhance service quality.
- Support and participate in innovation and evidence-based research.
